Privacy STATEMENT
PivOT Health takes your privacy and personal information very seriously
PivOT Health's Privacy Statement complies with the legislation outlined in the Privacy Act 1988. The information you provide to us is confidential and securely stored.
To ensure we comply with Victorian Healthcare Standards, we need to be able to identify you with at least 3 points of identification. This includes:
- Your name
- Date of birth
- Phone number
We also need your background social and medical history to ensure we can provide you with individual therapy and guidance that can specifically help you. Additionally, we will ask for your contact details to ensure we can communicate via email and/or phone.
We will NEVER share your information without your consent
You may wish for us to discuss your situation with your employer, education facility or health care team. We will never do this without you first requesting this and with your full knowledge and informed consent.

CANCELLATION POLICY
Please notify us by 9am on the day of your appointment
At PivOT Health, we understand that sometimes life changes and therefore appointments need to as well. Please provide as much notice as possible and at a minimum, before 9am on the day of the appointment to avoid a cancellation fee.
